A Hohmann retractor pulls back bone and tissue when you're working on joints and bones, but a Meyerding one is built for spine surgery where you need to hold back muscles really deep inside. Think of it this way one is for surface bone work, and the other one reaches down deep to get at the spine.

The blade has little teeth on it that grab onto muscles so it doesn't slip off while you're pulling back. Those teeth basically bite into the muscle and keep everything in place, making your job easier and keeping the surgical area open and clear.
